<p>「打瘦瘦針後視線變模糊，是藥物對眼睛有害嗎？」<br />「聽說腸泌素會讓人失明，是真的嗎？」<br />這類傳言近期在網路上廣為流傳，甚至讓不少用藥民眾產生恐慌。</p>
<p>蒼嵐健康美學診所院長蒼藍鴿（吳其穎醫師）表示，確實有研究指出，<strong>極少數使用腸泌素類藥物（GLP-1受體促效劑）的患者出現視力問題</strong>，其學名為「非動脈性前部缺血性視神經病變（NAION, Non-arteritic anterior ischemic optic neuropathy）」。</p>
<p>非動脈性前部缺血性視神經病變（NAION）可以把它想成「視神經的突發性供血不足」。視神經頭前端的血流被短暫阻斷或減少，導致視神經細胞受損。多半在剛睡醒時發生，患者會突然覺得一眼視力變模糊、視野出現黑影、對比度下降，但通常不會疼痛。</p>

<p>「醫師，我打猛健樂的時候瘦得很好，但停藥後就漸漸復胖，為什麼？」<br />這是減重門診常見的問題之一。蒼嵐健康美學診所院長蒼藍鴿（吳其穎醫師）指出，很多人在停用猛健樂（Mounjaro）或週纖達（Wegovy）這類腸泌素藥物後，體重迅速反彈，關鍵就在於「少做了一件事」&mdash;&mdash;<strong>沒有正確的慢慢減藥。</strong></p>

<p>減重藥物在控制食慾、穩定血糖、增加飽足感上效果顯著，但不能體重一達標就立刻停藥。<br />蒼藍鴿醫師說明：「如果沒有經過漸進式的減藥過程，人體的『體重設定點』尚未下降 ，代謝模式也尚未調節完成，體重就很容易反彈。」</p>
<p>「體重設定點」是減重醫學的重要理論之一。例如一個長期維持在80公斤上下的人，體重設定點就會在80公斤左右。一旦在短時間內減重10公斤，但體重設定點尚未改變的情況下，停藥後就容易漸漸復胖回80公斤。</p>
<p>「讓體重設定點往下的關鍵，就是<strong>在目標體重停留越久越好，盡可能2-3個月以上</strong>，如此可以將復胖的風險與速度降到最低。」減重專科蒼藍鴿醫師解釋。</p>
<p><strong>正確的減藥步驟舉例：</strong></p>
<ul>
<li><strong>先減少劑量：</strong><br />例如原本每週注射 5mg，可先調成 2.5mg，讓身體逐步適應。</li>
<li><strong>再減少注射頻率：</strong><br />例如原本每週注射一次2.5mg，可先改為每兩週一次，再慢慢延長間隔。</li>
</ul>
<p>透過這樣的「漸進式減藥」，可以讓身體在不依賴藥物的情況下，維持新陳代謝與體重平衡，同時也讓體重設定點慢慢下降。當然每個減重個案情況不同，還是建議跟醫師討論，找到最適合自己的減藥模式。</p>

<p>蒼藍鴿強調：「血糖藥突然停掉，血糖可能會反彈，造成血管的傷害；血壓藥也是，突然停藥可能會導致血壓驟升。同理可運用在減重藥上，如果體重達到目標，務必要<strong>漸進式的減藥</strong>，才能將反彈風險降至最低。」</p>
<p>其實，<strong>肥胖就是一種慢性病</strong>，當生活型態不足以讓體重有效下降時，藥物就是輔助療法之一。許多客戶在使用減重藥後，不僅瘦下來，<strong>連血糖、血壓、血脂也一起改善</strong>，反而能減少其他慢性病藥物的用量。</p>

<p>此外，他亦提醒，減重治療的成功不僅僅倚賴藥物，更需搭配規律運動、均衡飲食與充足睡眠等生活型態改變。藥物可以成為短期介入的輔助工具，但若無整體生活習慣的調整，仍難以達成長期健康管理目標。</p>
